丰田生产方式之父 大野耐一 有句名言:遇到问题,要对着它连问五次"为什么"。他举的就是机器停机的例子 ——
停机是因为保险丝断,保险丝断是因为轴承没润滑,没润滑是因为油泵不抽油,不抽油是因为泵轴磨损,
泵轴磨损是因为滤网坏了混进铁屑。到第五个为什么,真凶才现身。若只换保险丝,几天后照停;
装个滤网,问题才永远消失。大野耐一说:不重复问为什么,就不可能挖到表象之下的真正原因。
Taiichi Ohno, the father of the Toyota Production System, had a famous line: when a problem hits, ask "why" five times in a row. His textbook example is a stopped machine —
it stopped because the fuse blew; the fuse blew because the bearing wasn't lubricated; it wasn't lubricated because the oil pump wasn't pumping; the pump wasn't pumping because the pump shaft was worn;
and the shaft was worn because a broken filter let metal chips into the oil. Only on the fifth "why" does the real culprit appear. Swap the fuse and the machine stops again in days;
fit a proper filter and the problem is gone for good. Ohno's lesson: without asking "why" again and again, you simply cannot reach the cause that lives underneath the symptom.

在浅层下对策(换保险丝)成本低却治标不治本,问题会反复回来;在根因层下对策(修滤网)一次性根除。这就是 A3 报告"分析栏 → 对策栏"的逻辑。 A countermeasure at a shallow layer (swap the fuse) is cheap but only treats the symptom — the problem comes back. A countermeasure at the root layer (fix the filter) eradicates it once and for all. This is exactly the logic that links the "Analysis" box to the "Countermeasures" box on an A3 report.
3 现实里的 5 Why The 5 Whys in the Real World
根因分析 RCA:质量事故、设备故障、安全事件,先连问为什么找真因,再谈对策。
Root cause analysis (RCA): for quality escapes, equipment failures and safety incidents, ask why repeatedly to find the true cause before discussing countermeasures.
A3 报告分析栏:A3 的"根因分析"格子就是 5 Why 的栖身之所,对策必须对准最底层那个为什么。
A3 report "Analysis" box: the root-cause analysis pane on an A3 is the natural home of the 5 Whys, and every countermeasure must aim at the bottom-most "why."
安灯停线后:丰田拉绳停线,班组长第一件事就是带着大家现场连问为什么,当场堵漏。
After an andon pull: at Toyota, when the line stops, the first thing the team leader does is walk to the spot and run the team through the "whys" right there to plug the leak on the floor.
配鱼骨图用:鱼骨图横向铺开所有可能原因,5 Why 沿一根骨头纵向钻到底,两者互补。
Paired with the fishbone diagram: the cause-and-effect (fishbone) diagram spreads every possible cause sideways; the 5 Whys then drill vertically down one bone to the bottom. The two are complementary.

人遇到问题的本能是急着灭火,对着第一眼看到的症状就下手 —— 结果按下葫芦浮起瓢。
5 Why 的全部价值,就是逼你在动手前多问几层,把对策从"拍症状"升级成"断根"。
"五"不是教条,是一种不许停在第一个答案的纪律:可能三次就到底,也可能要七次。
判定标准只有一个 —— 这个原因消除后,问题不再复发,那它才是根因。
The human instinct under pressure is to rush in and put out the fire, swinging at the first symptom in sight — and that's exactly how problems keep popping up somewhere else.
The whole value of the 5 Whys is that it forces you to ask a few more layers before acting, upgrading the countermeasure from "swatting symptoms" to "cutting the root."
"Five" is not dogma. It is a discipline of refusing to stop at the first answer: sometimes three "whys" are enough, sometimes seven aren't.
There is only one acceptance test — if removing this cause keeps the problem from coming back, it is the root cause.
常见误用
Common Mistakes
问到第一层就动手(换保险丝了事)。钻到"消除后不复发"那一层才算到底。
Acting after the first "why" (just swap the fuse and call it done). You're only at the bottom when removing the cause stops the problem from coming back.
追问追成"追责"(为什么是人的错)。对准流程与系统,别对准个人,否则没人敢说真话。
Turning the questioning into a blame hunt ("why is this someone's fault?"). Aim at the process and the system, never at the individual — or nobody will ever tell you the truth again.
不看现场凭空猜五层。每一层都要有现地现物的证据支撑,否则只是脑补。
