丰田生产体系的工程师 新乡重夫(Shigeo Shingo)有个洞见:人犯错是必然的,但缺陷不是必然的。
靠"注意力""责任心"去堵漏,注定按下葫芦浮起瓢。于是他提出 poka-yoke——日语"防呆、避免疏忽",
核心是把防呆设计进装置本身:零件方向不对就装不进、漏装一个就触发传感器、形状对不上就卡住。
质量管理从此完成一次跃迁 —— 从"事后检测错误",升级为"源头预防错误",让良品成为唯一可能的结果。
Toyota Production System engineer Shigeo Shingo had one insight: humans will make mistakes — but defects don't have to follow.
Plugging leaks with "pay attention" and "take responsibility" is whack-a-mole. So he coined poka-yoke — Japanese for "avoiding inadvertent errors" —
and put the protection inside the device itself: a part that's facing the wrong way won't seat; a missing screw trips a sensor; an out-of-shape piece jams.
Quality management took a leap — from "inspect errors after the fact" to "prevent errors at the source", making "good" the only possible outcome.

上条是当前"靠人小心"会流出的缺陷率(随速度上升而恶化),下条是当前"靠防错"的实际流出率(≈0)。差距就是防错创造的价值。 Top bar: defect rate when you "rely on people being careful" (worse as speed climbs). Bottom bar: actual rate "with mistake-proofing" (≈0). The gap is the value poka-yoke creates.
3 现实里的防错 Poka-Yoke in the Real World
形状防呆:USB、SIM 卡、电源插头做成非对称形状,插反就是装不进,从根上杜绝接反。
Shape interlocks: USB, SIM cards, power plugs use asymmetric shapes — backwards simply won't seat, killing the wrong-orientation error at the source.
传感器防错:装配工位用光电/称重传感器,漏装一颗螺丝就报警停线,缺陷无处遁形。
Sensor-based mistake-proofing: photo-eyes and load cells at the station — miss one screw and the line alarms and stops. Defects have nowhere to hide.
定位夹具:工件只有一个正确朝向能放进夹具,方向错了物理上放不下,靠手感的活变成防呆。
Locating fixtures: the part only seats one way — wrong orientation physically won't fit. A "feel for it" job becomes a foolproof one.
流程防错:ATM 先退卡再吐钱、表单必填项不填不让提交 —— 把"容易忘"的步骤变成"想跳过都不行"。
Process mistake-proofing: an ATM returns your card before dispensing cash; a form won't submit until required fields are filled. The "easy to forget" step becomes "impossible to skip".

防错的精神,是把质量责任从"人要更努力"转移到"系统要更聪明"。
它有三个层级:控制级(错了根本动不了,最强)>警告级(错了会提醒,但仍可继续)>检知级(出错后能查出来,最弱)。
能做成控制级就别只做警告。它和下一站的自働化、安灯是一条心 —— 不让缺陷流到下道工序。
但记住:防错治的是无心之失(slip),治不了认知错误(如设计本身就错),后者得靠标准与培训。
The spirit of poka-yoke is to move quality accountability from "people must try harder" to "the system must be smarter".
Three tiers exist: the control method (error makes the step impossible — strongest) > the warning method (it alerts you, but you can still proceed) > detection (you can catch the error after the fact — weakest).
Reach for control before settling for warning. It pairs with the next page — jidoka and andon — sharing one principle: never pass a defect downstream.
One caution: poka-yoke fixes slips (inadvertent errors), not cognitive mistakes (a wrong design from the start). Those belong to standards and training.
常见误用
Common Mistakes
用"加强培训、提高责任心"替代防错。人必然会犯错,应把防呆设计进装置,而非寄望注意力。
Substituting "more training, more accountability" for mistake-proofing. Humans will err — build the protection into the device, don't rely on attention.
只做检知级(事后挑出来)就收工。尽量做到控制级(错了根本动不了),从源头杜绝。
Stopping at detection (catching defects after the fact). Push to the control method (the wrong step simply won't run) and kill the error at the source.
指望防错解决一切质量问题。它只防无心疏忽,设计缺陷和认知错误要靠别的手段。
Expecting poka-yoke to solve every quality problem. It only blocks slips. Design defects and cognitive mistakes need different tools.
