Goldratt 在《目标》里给出五步聚焦后,特意加上最关键、也最容易被忽略的第五步: 「如果前面的步骤打破了约束,回到第一步」,并郑重警告——不要让惯性成为系统的约束。 意思是:当你费尽力气突破了一个瓶颈,系统产出会上一个台阶,但必定有新的瓶颈冒出来接管限制。 如果你抱着「我们早就解决了那个问题」的旧规则、旧政策不放,改善就会卡死在那里。POOGI(持续改善流程) 就是把这五步固化成一个不停转动的循环——它不是一个项目,而是一种永远在路上的工作方式, 对应精益里「追求完美」那条永无止境的螺旋。 After laying out the five focusing steps in The Goal, Goldratt tacked on what he called the most critical — and most often forgotten — fifth step: "If a constraint has been broken in the previous steps, go back to step one," with a stern warning — don't let inertia become the system's constraint. His point: the moment you crack a bottleneck, throughput jumps a level, but a new bottleneck always shows up to take over the limit. Cling to "we solved that problem long ago" — to old policies, old rules — and improvement seizes up right there. POOGI hard-wires those five steps into a never-stopping loop; it isn't a project but a permanent way of working, the TOC mirror of Lean's never-ending spiral of pursuit of perfection.

① ~ ④ 突破当前约束:识别它、把它榨干、让全系统配合它、再花钱扩它的能力——直到它不再是瓶颈。 Steps ① – ④: break the current constraint — name it, squeeze every drop from it, make the rest of the system march to its drum, then spend money to expand it — until it stops being the bottleneck.
⑤ 回到第一步:约束一旦被打破,新约束立即出现。绝不能停,更不能让「旧成功的惯性」变成新的约束。 Step ⑤: go back to step one. The instant a constraint breaks, a new one appears. Never stop — and never let the inertia of yesterday's win turn into tomorrow's constraint.

Goldratt's line — "don't let inertia become the constraint" — is worth its weight in gold: the most dangerous bottleneck is rarely a machine, it is the unchallenged rule "we've always done it this way". The whole point of POOGI is to force the organisation to keep its eye on the next constraint and keep spinning the next lap.
